Performed individual home modification assessments to assist consumers to remain living in their homes by providing attendant care evaluations, peer counseling, design and product solutions, and housing counseling services, as approved through the U.S. Department of Housing and Urban Development. Assisted homeless individuals in seeking immediate temporary and long-term accessible housing.
